Coverage That's There When You Need It

When you're a USA Lacrosse member, you get more than access to the game. You get built-in coverage and resources designed to help keep players, coaches, officials, and volunteers protected every step of the way.

Your membership includes:

  • Accident medical coverage for injuries that occur during organized lacrosse activities
  • Liability protection if you're sued for causing an injury or property damage
  • Game-fee reimbursement for officials who can't work due to an injury suffered while officiating
  • Liability coverage for program leaders and volunteers in programs where all players and coaches are active members of USA Lacrosse 

Accident Claim

How to file an accident claim

If you are a current USA Lacrosse member who is injured during a covered lacrosse activity, you may file a claim online by clicking here

Note: USA Lacrosse members selecting this option will be redirected to the A-G Administrators website to complete the accident claim form.
 

Liability Claim

How to file a liability incident claim

The online liability claim form below should be used by USAL member organizations when a general liability incident (property damage or bodily injury during a covered lacrosse activity) has occurred.

If you are a USAL player, coach or official member, and have been injured during a covered lacrosse activity and would like to file a personal injury claim, please use the Accident Claim Form linked above.

 
 

Certificate of Insurance

How To Request a Certificate of Insurance

Step 1. Login to your USA Lacrosse account, go to the Program Compliance section, and upload a roster from the most current season if not done previously. If you are a Program Leader and you do not see your program as an option to select when you login to your personal USAL account, please contact RegionalTeam@usalacrosse.com.

Step 2. After completing step 1 (roster upload), click on the “Member Org Agreement” button, review the terms of the agreement, and select the checkbox to agree to them. You can click on the “Member Org Agreement” button again at any time to review, download and print this agreement.

Step 3. Click on the “Request Certificate of Insurance” button to be redirected to the request form. You may request another Certificate of Insurance as many times as needed.
