Regional Stick Grant Agreement

We are excited to hear you are ready to receive your stick grant from USA Lacrosse (“USAL”). While this stick grant will serve your youth community to develop the sport of lacrosse, we also know many youths will move on to adopt the sport through this introductory program.

In order to maintain consistency in the stick grant program (“Program”) and gather the data that will serve to support this program into the future, all Pre-Approved Stick Grant Qualifying Organizations (“Host”) shall agree to the following terms and conditions:

  1. Host shall have no rights to USA Lacrosse intellectual property, including but not limited to logos and name with the exception of the following statement, “The sticks have been donated by USA Lacrosse.”;
  2. Host shall maintain an active USA Lacrosse Member Organizational status, while hosting stick grant programs;
  3. Host shall ensure that all coaches maintain an active bronze level USA Lacrosse Membership, while hosting stick grant programs;
  4. Host shall maintain the status of a non-profit entity in their jurisdiction while hosting stick grant programs;
  5. Host shall submit a copy of their completed W9 upon execution herein;
  6. Host shall request non-member insurance event coverage through their Regional Manager. The host must submit a request form for each event where non-members will be present;
  7. Host shall set up a program that utilizes their own registration process for participants;
  8. Host agrees not to charge participants in the initial event;
  9. Host agrees to solely utilize their own balls that are either low weight pinkie type balls, or tennis balls;
  10. Host agrees to conduct introductory clinics throughout the year;
  11. Host agrees to complete the USAL "Member Organization Stick Tracking Form" after conducting their introductory clinic and throughout the year if applicable.
  12. Host agrees to abide by USAL’s Native American, Moral and DEI Standards stated below;
  13. Host agrees to lend each participant one stick per event, for the length of the event, before returning to Host; and
  14. Host agrees to maintain a safe secure location to store sticks in between events.

Host understands that failure to meet these requirements will sacrifice any future involvement in USAL grant programs and require the Host to ship any and all sticks provided under the Program back to USAL at their own expense within 30 days of notice.


Assumption of Risk           
I understand that there are certain risks of injury that may arise from the possession, use or misuse of the Equipment, including the risk of injury, disability, or death, or damage to property. I assume full responsibility for all risks arising directly or indirectly from my possession, sue or misuse of the Equipment, both known and unknown, regardless of the cause.

Waiver and Release             
I waive and release any and all claims against USAL, its agents, employees and elected officials (collectively “USAL Parties”) with respect to any and all injury, disability, death, loss and damage to property resulting from possession, use or misuse of the Equipment, regardless of the cause and even if caused by negligence, whether passive or active. I agree not to sue any of the USAL Parties on the basis of these waived and released claims.

Indemnity             
I will defend, indemnify, and hold the USAL Parties harmless from and against any and all liability, loss, damages, claims and attorney’s fees that may be suffered by any USAL Party resulting directly or indirectly from the possession, use or misuse of the Equipment by me or any other person.

Native American, Moral, and DEI Standards            
USAL is committed to diversity, equity, and inclusion as core components of its mission to fuel the sport’s national growth and development, and seeks sponsors, partners and vendors that share this priority. To ensure alignment, USAL may request that its sponsors, partners and vendors confirm their commitment in this regard by submitting for consideration relevant documentation of corporate philosophy, policy and practice for review, including representative examples of marketing content that incorporates inclusive language and imagery at any time. In addition, USAL sponsors, partners and vendors agree to not, in any aspect of operations including employment, discriminate against any individual on the basis of race, color, sex, age, national origin, religion, marital status, disability, sexual orientation, gender identity and any other classifications protected by local, state, and federal law.
