USA Lacrosse (USAL) understands that it can be difficult to report suspicions or allegations of misconduct towards athletes (especially child abuse or neglect, including child sexual abuse), and respects the sensitive nature of all reports. When filing a report with the USA Lacrosse Athlete Protection & Safety Program, you are encouraged to provide as much information as possible as it helps to effectively respond to the reported misconduct.

You may report anonymously by omitting any identifying information. However, please be aware that this may limit USAL’s ability to respond to the information you provide. If you wish to report anonymously, either leave identifying information fields blank or put "Anonymous". A copy of this report will also be sent to the email provided in the background information field for your records.

Knowingly making a false or vindictive report will not be tolerated and can result in termination of your USA Lacrosse Membership.

For example, who was involved in the alleged misconduct, what was the alleged misconduct that occurred, when did the alleged misconduct occur (date, time) and where did the alleged misconduct take place (state, city/town, specific location).

By law, any suspicions or allegations of child abuse or neglect (including child sexual abuse) must be reported to law enforcement and child protective services within 24 hours. USAL policy requires that all staff members, volunteers, and members of USAL must report any suspicions or allegations of child abuse or neglect (including child sexual abuse) to law enforcement and child protective services and notify the USAL Athlete Protection & Safety Program.
